Please indicate the event you are purchasing a ticket for.
- Give the names of all individuals for whom you are reserving and paying.
- For online events (Zoom), include your e-mail address and that of any others you are reserving for.
- If you are adding a certain amount as an extra donation, or if you are paying a membership fee at the same time, state that clearly and indicate how much of the check should be applied to those uses.
